Anyone under 18yrs, will need to have their parent/guardian's approval to submit for this project. I (parent/guardian) hereby confirm that I am submitting my minor child for consideration in this Imaginative Kids video project. I understand our personal information, including self-recorded video and/or interview, may be shared with third parties for the purpose of casting this project. If my child is selected for the project, I agree to allow him/her to participate and understand that they will be compensated for their participation. The project is currently scheduled to shoot TBD early November 2025 and I understand my child will have to miss school in order to participate.
